I am sure every experience is different for every traveler, but this is the first travel pillow that has worked for me. I am tall and have constant neck pain, yet this allows me to comfortably rest and even sleep on flights. It does not depend on the back rest extending or not and I found it versatile and easy to travel with. The only word of caution is that it requires wrapping a fleece around your neck (and part of face) which is HOT! For me it was acceptable on flights with good air flow, but I couldn't use it while on the ground before the AC kicks in because of this. Likewise, I couldn't use it on trains because of the heat. I think there is aa lightweight version. I wish I'd found that first and recommend that if you are prone to run hot. It's too expensive to buy a second one though....although I'm really considering it.
